The Chronic Disease Management Programme, or CDMP, was established in October 2006 with the intention of improving the level of care for chronic disease patients and making treatment more accessible and affordable for Singaporeans. This is an effort to improve the overall management of chronic disease in the country. Besides chronic physical illnesses, the CDMP also covers several mental illnesses as well. 

The following conditions are currently covered under the CDMP.

From 1 Jan 2021, patients with complex chronic conditions will be able to use up to $700 per patient each year, while other patients will be able to use up to $500 per patient yearly for their treatments. Each MediSave claim is subject to a 15% co-payment in cash. Find out more regarding MediSave here. Patients can keep track of the amount of MediSave used under this programme in a separate account summary in your CPF statement on the CPF website. Participating clinics can also check the MediSave balances under the CDMP on behalf of patients via the Mediclaim system. Patients with chronic conditions under the CDMP can also tap on government subsidies of up to $500 per year under the Community Health Assist Scheme (CHAS) if they are CHAS cardholders, and special subsidies of up to $540 per year if they are Pioneer Generation or Merdeka Generation cardholders. Each claim is subject to a 15% co-payment in cash. This is regardless of whether the bill is for a one-off visit or a package.

From 1 Nov 2009, Medisave can also be used for pneumococcal vaccinations for children under the age of 6 years and Hepatitis B vaccinations, subject to the same withdrawal limit of $400 per Medisave account per year. 

In line with the expansion of CDMP, Pioneers and Community Health Assist Scheme (CHAS) cardholders will be able to tap on their prevailing CHAS Chronic subsidies (of up to $540 and $480 per year respectively with effect from 1 June 2018) for the management of pre-diabetes mellitus (DM) and Ischaemic heart disease (IHD). Those who are eligible may also tap on prevailing CHAS Chronic subsidies to defray the costs of glucose test strips and lancets.
