Purpose & Vision:

Family doctors in the community delivering preventive care have to consider the varied care needs of the population, and that the approach for preventive care and various chronic diseases needs to be effective, consistent and well-coordinated. To address this, a clinical service roadmap for primary care is being developed with healthcare clusters, leaders from the College of Family Physicians Singapore (CFPS), Singapore Medical Association, Primary Care Networks (PCNs), and the National General Practitioner Advisory Panel. It will be a multi-year effort, with clear priorities at each stage.

The roadmap will cover care protocols that include (1) preventive care, (2) chronic care, and (3) administrative care protocols. The care protocols provide clear and concise guidance for assessing and managing a patient for a particular condition. The content of each care protocol is jointly developed with family doctors, healthcare clusters, clinicians and other healthcare professionals, and taking into consideration the relevant national clinical practice guidelines (CPGs) and Agency for Care Effectiveness (ACE) guidelines.

The care protocols will encompass the following sections:

  5. Data Submission
  6. References

The Care Protocols also cover recommended health screenings, medications, lifestyle adjustments, and when to escalate to specialist and acute care. This will assure residents that critical care elements are consistently carried out regardless of which doctor they choose to enrol with under Healthier SG.

Subsequently, the care protocols will progressively cover other complex chronic conditions, such as mental health and end-of-life care. The roadmap will be reviewed periodically with family doctors and healthcare clusters to ensure continued relevance.
